The Healthiest You Diabetes Management program
The Healthiest YouTM Diabetes Management Program is a virtual program that combines doctor-led treatment plans, Certified Diabetes Educators (CDE) assessments and workshops, individual counselling, and group education sessions.
We provide personalized and sustainable care for long-term results. In addition to medication and provider support, our program provides science-backed educational resources to help you make meaningful changes in your diabetes management. Program topics include:
